Position Overview

The purpose of the ApartmentConnect Lead is to assist tenants that have mental health concerns and/or substance use disorders, physical health needs, and barriers to adequate food or transportation while leading the team of Specialists. The ApartmentConnect Lead will be working on-site at the apartment complex(s) to provide in-person support to the tenants while leading the team of Specialists located at the additional sites as needed. The services offered may be broad or intensive and include service coordination with Dakota County Social Services, contracted case management, mental/chemical health services, public assistance and benefits programs, community organizations, as well as providing assistance with paperwork and tenant issues. The ApartmentConnect Lead will maintain and expand relationships built with Dakota County, property management, and community organizations who we partner with in the community.

Role Responsibilities

  • Establish trusting relationships with tenants so they feel comfortable accepting services.
  • Prioritize exploring creative solutions to maintaining housing for all tenants. When a disruption does occur, connect tenants to a better suited living environment to reduce the risk of homelessness and the strain on Dakota County Social Services (DCSS), community organizations, and other homeless response services.
  • Respond to referrals from property managers to provide a timely follow-up contact with the tenant to determine if the individual/household is interested and/or eligible for voluntarily programs offered through Dakota County or a community-based organization.
  • Link or refer tenants to programs and services included in the service grid to promote housing stability and affordability.
  • Responsible for maintaining partnerships and relationships with community organizations to bring services and/or events on-site as appropriate and approved by property management. Including but not limited to food access programs, Libraries, Parks and Recreation, etc. on a quarterly basis.
  • Learn about the mental health crisis and other services offered by DCSS, including the roles and responsibilities of the 24 hour, seven days a week, Crisis Response Unit (CRU) and on-going mental health programs and utilize them as needed.
  • Utilizing the Housing Portal provided by Dakota County to track the required demographic information asked for in the application. Additionally, maintaining an organized and updated list of active clients on the Housing Portal.
  • Provide support and leadership to the ApartmentConnect Specialists when needed to ensure services are being delivered effectively.
  • Coordinate with your assigned system partners: Embedded Social Worker and their Law Enforcement Partner as well as the Multi-Family Officer.
  • Respond in accordance with RTI’s policies, when available and assistance is requested, to a mental health crisis or emergency in coordination with Law Enforcement, Crisis Response Unit, and/or the Embedded Social Worker.
  • Review police department services calls monthly and identify methods to decrease the number of non-criminal service calls to apartment complex through referrals for ongoing support services necessary to prevent mental health and physical health crisis and to encourage preventative engagement.
  • Submitting monthly and quarterly reports to Dakota County and RTI on program outcomes, social services referrals made, incidents occurred, and satisfaction of participants and stakeholders.
  • Other tasks as assigned by Department Director.

Required Qualification and Experience:

  • Associate degree in related field of behavioral health OR Has 4,000 hours of supervised experience in Housing or working directly with individuals experiencing severe and persistent mental illness (SPMI).
  • 18 years of age or older with a completed High School Diploma or it’s equivalent.
  • Ability to pass a Department of Human Services background check.
  • Must have access to a reliable vehicle and maintain a valid driver’s license and insurance
  • Ability to pass the BCA Criminal Background Check and the Sexual Exploitation of Patients by Psychotherapist screen required by DHS.
  • Ability to pass screening required by Federal Human Services office of the Inspector General
  • Ability to organize and work independently.
  • Possess a person-centered approach, consistent with the Philosophical approach of the individual program and RTI
  • Ability to work with culturally diverse populations.
  • Other, as needed, as company and role expand or evolves.

Preferred Qualification and Experience:

  • Minimum 2 years of housing experience.
  • Minimum 1 year of supervisory experience.

Compensation: $25.50 per hour

Physical & Mental Requirements

Employees in this role must be able to:

  • Walk, stand, sit, bend, climb stairs, and perform crisis prevention/intervention techniques
  • Use hands for writing, typing, and data entry
  • Lift up to 40 lbs. and occasionally exert physical effort during duties
  • Maintain vision and hearing necessary for reading charts, operating office equipment, using a computer, and driving a vehicle
