Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patients’ health by taking vital signs such as blood pressure, pulse, and temperature; report any changes to healthcare professionals
- Administer medications accurately according to care plans while adhering to HIPAA regulations for patient privacy
- Prepare nutritious meals and assist with meal planning to meet dietary needs and preferences
- Support patients with cognitive impairments such as Alzheimer’s disease or dementia through memory care techniques and patient observation
- Use specialized equipment like Hoyer lifts or resident lifts safely to transfer clients with mobility challenges
- Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment in compliance with health standards
- Document care activities thoroughly and update care plans regularly to ensure personalized support for each resident or client
- Provide emotional support and companionship, fostering a positive environment that promotes dignity and independence
- Collaborate with healthcare teams, social workers, and family members to coordinate comprehensive care strategies
Qualifications
- Proven experience in home health, assisted living, nursing homes, or group home settings is highly desirable
- Knowledge of patient observation techniques and ability to monitor vital signs accurately
- Certification in CPR and First Aid is required; additional certifications such as HIPAA training are a plus
- Experience working with individuals with developmental disabilities, autism spectrum disorder, or memory-related conditions like Alzheimer’s or dementia is preferred
- Ability to lift heavy objects safely and perform physical tasks associated with patient transfers using equipment like Hoyer lifts
- Strong communication skills to effectively interact with clients, families, and healthcare professionals
