Overview
This is an opportunity for a dedicated Registered Nurse Case Manager to join our team and make a difference in the lives of patients in hospice care. You will be part of a supportive team, providing essential care and coordination for patients and their families.
Open Locations:
Elmhurst, Evanston, McHenry, Mount Prospect, Northbrook and Wheeling IL
Key Responsibilities
- Manage patient care in hospice settings
- Coordinate with healthcare teams
- Provide leadership and support to nursing staff
Top Skills Required:
- 1 year of professional experience as an RN (Home Health experience preferred)
- Active RN License in the State of IL
- Associates Degree (or higher) in Nursing
- BLS and CPR Certification (if you don't have this - we will help you get it!)
- Valid Driver's License, Reliable Mode of Transportation, and Auto Insurance
What you need to know:
- Fulltime | Monday - Friday
- Flexible schedules, no long shifts, and minimal weekend shifts a year
- A comprehensive 6-week RN onboarding program
- Full Benefits (Health, Dental, and Vision)
Comp Range:
- Salary - $80k to $85k $10k FYS Bonus
- Mileage reinbursement
- Company Vehicle available after 6 months.
Schedule:
Hours: Fulltime | 40hrs per week
- Monday - Friday 8-5pm
- Make your own hours (flexibility on when to accept patients and visit them)
- Schedules are build in advance
- RN's will have a scheduling assistant.
- (2) weekdays of 5P to 8A
- (2) holidays per year – 1 summer & 1 winter; 3 floating holidays
Qualifications:
- Hospice (1 years)
- 1 years of experience in ANY of the following:
- Valid Registered Nurse (RN) in the state of Illinois (IL)
- Life Support (AHA BLS) or American Heart Association Advanced Cardiac Life Support (AHA ACLS)
- Home Health Care
- Epic Home Health
- Home Health
- Management (1 years)
- Valid Driver's License, Reliable Mode of Transportation, and Auto Insurance.
