Job Description

At Residential Home Health and Hospice, we're looking to add to our extraordinary care team. Our home health registered nurse case managers lead and collaborate to provide management and delivery of patient care plans.

About the Role

  • Support patients in Port Huron and surrounding areas
  • Eligible for a $10,000 first year success bonus
  • Up to 23 paid holiday and personal days off in year one
  • Monthly clinical outcome bonuses after one-year of employment
  • Dedicated schedulers for flexible scheduling options
  • A company provided tablet and smartphone with 24/7/365 IT support

Salary : $80,000 - $140,000
