What are the responsibilities and job description for the Patient Transition Coordinator position at Residential Home Health and Hospice?
The Patient Transition Coordinator will initiate contact with patients to assist in preparation for accepting care. This includes obtaining demographic information, medical history, and primary care physician details.
- Introduce the company's Home Health services to patients/caregivers, explaining benefits and coordinating clinical information.
- Provide feedback to referral sources regarding admissions and non-admit decisions.
- Collaborate with rehabilitation facilities on patient care needs and potential service needs at discharge.
This role requires excellent organizational skills, high energy, and a passion for care delivery. Proficient computer skills are necessary, including Microsoft email, word, excel, and agency software. Experience in a healthcare setting and sales experience are preferred.
Requirements:
- Demonstrated knowledge of referral source types and community resources.
- Valid Pennsylvania Driver's License, automobile insurance coverage, and access to a reliable automobile.
Salary : $40,000 - $60,000
