Responsilibities

  • Deliver skilled physical therapy and rehabilitation therapy in patients' homes
  • Develop and implement individualized treatment plans.
  • Collaborate with an interdisciplinary care team.
  • Document patient care using EMR systems (HCHB)

Qualifications

  • Licensed Physical Therapist in Pennsylvania
  • Recent graduates are encouraged to apply; this role offers opportunities for learning and professional development
  • Valid driver's license and reliable transportation

Low end of the range is the average annual compensation earned based on a fully productive caseload.
