JOB SUMMARYResponsible for the screening, monitoring, and oversight of all Healthy Alliance Social Care Network activities within the Rescue Mission of Utica.
ESSENTIAL JOB FUNCTIONS
- Conduct screenings and referrals of clients through UniteUs system.
- Responsible for monitoring and tracking of data related to Healthy Alliance services provided to clients
- Generate and submit client data reports to supervisor
- Coordinate incoming and outgoing referrals with program staff & supervisors
- Responsible for gathering and processing client data and documentation
- Conduct ongoing Healthy Alliance trainings with approved staff and supervisors
- Stay abreast of all updates and regulatory changes in the Medicaid Redesign Project on both federal and state levels.
- Build trust and rapport with clients of all Rescue Mission programs
- Abide by all Mission, program and safety policies, procedures and guidelines.
- Attend and participate in regular meetings and trainings as requested by your supervisor and Healthy Alliance.
- All other duties as requested by your supervisor.
- High School Diploma or equivalent.
- Minimum 2 Years experience in Human Services required
- Associates degree in related field preferred.
- NYS Drivers License required
- Familiarity with database usage and data entry.
- Experience with health management software
- Advanced skills with Microsoft Word, Excel, and Outlook.
