Our nationwide team of skilled wound care specialists partners with skilled nursing homes, assisted living facilities, and long-term acute care hospitals to provide advanced treatments and reduce hospital readmissionsimproving lives and outcomes every day.Were looking for passionate and proactive Nurse Practitioners to join our mission of healing and innovation.What Youll DoAdvanced Wound Care & Patient AssessmentConduct thorough assessments of chronic and acute wounds.Develop and implement individualized, evidence-based treatment plans.Apply advanced techniques such as debridement, dressing selection, skin grafts, and infection control.Collaborative Clinical LeadershipPartner with facility healthcare teams to coordinate care plans and treatment updates.Ensure continuity of care through consistent communication and collaboration.Multi-Facility Travel & SchedulingManage a travel schedule visiting multiple facilities weekly.Ensure timely and efficient care delivery with professionalism and compassion.What Were Looking ForValid Nurse Practitioner license (State-specific)ANCC or AANP Board Certification1 years of experience in advanced wound care highly preferredWound care certification (CWOCN, CWS, WCC, WOCN) is a plusReliable transportation and willingness to travel consistentlyPreferably licensed in Pennsylvania in addition to New Jersey.
