Renaissance Social Services, Inc. is seeking a compassionate, motivated, and dedicated case manager to join our supportive housing team.

Renaissance prevents and ends homelessness for vulnerable Chicagoans, providing a range of proven solutions including prevention, outreach, rapid re-housing, permanent supportive housing, behavioral health care, and clinical case management. 

 

JOB SUMMARY

The Clinical Case Manager for the Supportive Housing Outreach Program offers intensive clinical outreach and supportive services to participants diagnosed with mental health and substance use disorders, living in scattered site units throughout the city. The services provided address the psychosocial, health, financial, recreational, nutritional, and housing needs of clients. In this role, you will work closely with adults facing mental illnesses or substance use disorders, delivering supportive services that foster recovery, promote independence, and ensure housing stability.

 

ESSENTIAL DUTIES

  • Providing intensive mental health case management services in the community, including community building and social activity planning and implementation;
  • Identifying client and resident needs, including barriers to receiving appropriate services and assisting with effective problem solving to address issues;
  • Providing treatment planning, coordination, and implementation of services needed;
  • Providing information, linkage, referral and advocacy for clients and residents with agencies in the community, including crises intervention services when needed;
  • Working with clients and residents to increase their skills and income, improve their self determination, and maintain residential stability;
  • Maintaining the confidentiality of client information in any written, oral, or electronic form including, but not limited to, health information, HIV/AIDS status, and disability information. Releasing such information only as needed and with the proper release from the client unless mandated by law;
  • Completing submitting of billing sheets in accordance with Rule 132 guidelines;
  • Reporting all incidents of abuse, and/or neglect as required by state law;
  • Maintaining accurate and up-to-date records using HMIS and paper files on all clients and residents receiving services, including maintaining records necessary to fulfill grant requirements and third party payers;
  • Coordinating the provision of appropriate services from other providers from the community at buildings at which clients and residents are living;
  • Working cooperatively with building management to ensure residential stability of clients and residents;
  • Identifying and developing training opportunities for clients and residents;
  • Completing required documentation for Medicaid billing, HMIS and compliance in a timely manner.

Qualifications:

Minimum Requirements

  • Must meet the State of Illinois criteria for a Mental Health Professional (MHP) through one of the following pathways:

    • Bachelor’s degree in a human services field (Social Work, Psychology, Counseling, etc.).

    • Bachelor’s degree in an unrelated field plus two (2) years of documented clinical experience in a mental health setting.

    • High School Diploma or GED plus five (5) years of documented clinical experience in mental health or human services.

  • Proven experience working with individuals experiencing chronic mental illness and substance use disorders, with a strong foundation in Harm Reduction principles.

  • Demonstrated understanding of mental health symptoms, evidence-based treatment modalities, and community resource navigation.

  • Ability to professionally organize and document clinical progress notes and comprehensive service plans.

  • Strong computer literacy, including the ability to utilize Microsoft Office Suite and Electronic Health Record (EHR) systems for daily reporting.

  • Valid driver’s license and daily access to a reliable vehicle for community-based service delivery.

  • Must maintain active automobile insurance with minimum liability limits of 100/300/100.

 

Preferred Requirements

  • Master’s degree in Social Work, Psychology, or a related human services field.

  • Current Illinois professional licensure (LSW, LPC, LCSW, or LCPC).

  • Proven ability to work effectively as part of an interdisciplinary team, maintaining professional relationships with residents, families, and external community agencies.

  • Demonstrated ability to work independently, manage a caseload efficiently, and match services to the specific needs of residents.
