What are the responsibilities and job description for the Registered Nurse (RN) position at Renaissance Adult Day Health Care Center?
We believe the elderly and adults affected with cognitive, behavioral and/or physical disabilities deserve to have high quality care, as well as to function as independently as possible. Our participants receive quality nursing care, medication management, nutritious meals, outings, and a wide variety of stimulating activities tailored to participants' abilities. We are currently seeking a compassionate, dedicated and professional Registered Nurse (RN) to join our team on a part time basis.
The RN supports the program in implementing processes that address the health, psychosocial, and educational needs of older individuals and their caregivers and that provide clients a safe, secure, and positive daily experience.
- Coordinate with caregivers, families, social services agencies, and medical professionals to plan clients program of care. Monitor programs to ensure effectiveness, quality care, compliance with applicable regulations and standards, and to identify areas for improvement or expanded program offerings.
- Produce, maintain, and manage all clinical documentation. Record monthly progress notes (q30d) in participants' charts according to established policies and procedures as mandated by DHCF.
- Document and manage any incidents and/or significant changes in health status or condition.
- Manage participants' medications to ensure compliance, work closely with physicians and pharmacies, provide ongoing medication education to participants.
- Manage and maintain current physician's orders for each participant (q90d) as well as ongoing communication with participants' physician to document any changes in health status and to ensure ongoing quality nursing care.
- Lead the interdisciplinary treatment team meetings to develop and/or update individual plans of care for each participant (q90d). Implement and ensure compliance with prescribed treatments and approaches.
- Lead discharge planning in coordination with social worker and multi-disciplinary team.
- Monitor participant’s individual dietary and supplemental feeding plan as prescribed by the participants physician or medical director.
- Maintain adequate stock of nursing supplies, assist with in-service training, infection control, first aid, and other tasks as mandated by DHCF.
Home Health experience is a plus.
Relevant training is provided.
