What are the responsibilities and job description for the Virtual Nurse Coach position at Remote Care Partners?
The Role: Remote Nursing Health Coach
Key Job Functions:
- Welcoming patients to our programs
- Educating them on the equipment they will be using
- Near real time review of patient vital signs
- Obtaining actionable information from patients with abnormal readings
- Using practice EMR to escalate patient situations to the provider for medical intervention
- Creating strong patient relationships by engaging patients regularly, (minimum weekly), through calls, automated surveys, and video visits
- Monitoring notifications and observing trends
- Creating patient centered care plans as well as reviewing monthly with patients
- Monitoring patient compliance with the plan of care and devices
- Coaching patients on healthier behaviors
- Partnering with practice office staff to alleviate aspects of office workload
- Act as first contact for patient in non-emergency situations IE med refills, office appointment and lab scheduling
- Determine patient motivating factors and leveraging them for best possible health outcomes
- Proactively managing patients to prevent urgent care or emergency room visits
- Provides individualized virtual care for high-risk patients as well as ongoing chronic disease management
Required Experience:
- RN with 2 years nursing experience
- EMR Experience
- Knowledgeable with using computers
- Experience caring for some or all the following conditions:
- Hypertension
- Diabetes
- COPD
- Asthma
- CHF
- A Fib
- Active state licensure
- Self-starter mindset
Location: Remote, working from home
Job Details:
- This is a full-time, salary position
- Work hours to be determined upon practice needs
- Full benefit package
- Software and hardware will be provided upon hire
- Pay Range: 55k-60k
