Home Health RN Responsibilities

• Provide skilled nursing care in the patient’s home setting, including assessments, treatments, wound care, and medication management.

• Develop and implement individualized home health care plans, adjusting as patients’ conditions change.

• Educate patients and families on health conditions, treatments, and at-home care.

• Collaborate with the Director of Nursing, physicians, and interdisciplinary teams to coordinate quality care.

Qualifications for Home Health RN Jobs

• Graduate of an accredited nursing program (BSN preferred).

• Current California Registered Nurse (RN) license in good standing.

• At least 1 year of professional nursing experience (home health nursing experience strongly preferred).

• Public Health Nurse (PHN) certification a plus.

• Strong clinical judgment, communication, and organizational skills.

• Valid driver’s license, reliable transportation, and ability to travel to patient homes in Inglewood, Hollywood, Brentwood, Santa Monica and surrounding Los Angeles County areas.
