Responsibilities
- Assist residents with activities of daily living (ADLs), including personal hygiene, grooming, dressing, and mobility support using appropriate patient transfer methods such as Hoyer lifts.
- Monitor and observe patients for changes in condition, including vital signs, behavior, and overall health status, ensuring timely reporting to healthcare professionals.
- Support medication administration following prescribed care plans while adhering to HIPAA regulations to maintain confidentiality.
- Prepare nutritious meals and assist with meal planning and preparation tailored to individual dietary needs, including dementia, Alzheimer’s, or other memory care requirements.
- Maintain a clean and safe environment by performing routine cleaning tasks and ensuring compliance with health and safety standards.
- Provide transportation assistance for appointments or community outings, fostering independence and social engagement.
- Document care activities accurately in accordance with facility policies and assist in updating personalized care plans regularly.
- Engage residents in meaningful activities that promote mental stimulation, social interaction, and overall well-being.
Skills
- Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly desirable.
- Strong patient observation skills coupled with the ability to monitor vital signs and recognize signs of distress or deterioration.
- Knowledge of dementia care, Alzheimer’s care, memory care strategies, and neurodevelopmental disorders such as autism is essential.
- Proficiency in manual handling techniques including heavy lifting and patient transfer methods like Hoyer lifts.
- Familiarity with medication administration protocols and first aid/CPR certification to ensure safety during emergencies.
- Excellent communication skills for effective interaction with clients, families, healthcare teams, and community resources.
- Ability to follow detailed care plans while respecting residents’ rights and promoting autonomy.
- Experience supporting individuals with developmental disabilities or intellectual disabilities is advantageous.
