Home Health RN Responsibilities
- Provide skilled nursing care in patient homes, including assessments, wound care, medication management, and treatments
- Develop and implement individualized home health care plans
- Monitor patient progress and adjust care plans as needed
- Educate patients and families on disease management, rehab, and senior care
- Coordinate care with physicians, therapists, and the Director of Nursing
- Document patient visits in compliance with home health regulations
Qualifications – Home Health RN Jobs
- Active California Registered Nurse (RN) license (required)
- Minimum 1 year RN experience (home health experience preferred)
- Experience in rehab, geriatric care, or community health is a plus
- Strong clinical, communication, and organizational skills
- Valid driver's license and reliable transportation
- Ability to travel throughout South Bay Los Angeles (Torrance, Carson, Rancho Palos Verdes, El Segundo, Compton)
Preferred Qualifications
- Home Health RN experience
- Public Health Nurse (PHN) certification
- Experience with mobile therapy or home-based care models
