As a Home Health Occupational Therapist, you'll help patients:
- Regain independence through customized therapy plans
- Improve functional abilities for activities of daily living (ADLs)
- Recover from injury, surgery, or hospitalization in the comfort of home
- Reduce fall risk and improve home safety
- Performing in-home OT evaluations and reassessments
- Creating individualized care plans tailored to each patient’s goals
- Collaborating with nurses, physical therapists, and physicians
- Educating patients and caregivers on proper techniques and safety
- Documenting patient progress in compliance with state and Medicare guidelines
- Licensed Occupational Therapist in the state of California
- Graduate of an ACOTE-accredited OT program
- Minimum 1 year of clinical OT experience (Home Health experience preferred)
- Familiar with electronic documentation systems (EMRs)
- Excellent communication and time management skills
- Valid driver’s license, auto insurance, and reliable transportation
