Your Role: Home Health LVN

As a Licensed Vocational Nurse (LVN) in Home Health, you will deliver skilled nursing care to patients in their homes under the supervision of a Registered Nurse (RN). You’ll play a vital role in supporting patients through rehabilitation, chronic condition management, and senior care services.

Key Responsibilities

• Provide skilled home health nursing care based on individualized care plans
• Support senior care and rehab patients in home and community settings
• Document patient visits, treatments, and progress in EMR systems
• Collaborate with physicians, RNs, families, and interdisciplinary teams
• Assist patients with chronic conditions, recovery, and dementia care
• Ensure compliance with California home health regulations
• Promote patient safety, independence, and overall well-being
  
  

Qualifications

• Active California Licensed Vocational Nurse (LVN) license in good standing
• Graduate of an accredited vocational nursing program
• Minimum 1 year of nursing experience (home health preferred)
• Experience in senior care, rehab, or community care preferred
• Valid driver’s license, reliable transportation, and auto insurance
• Strong communication, organization, and documentation skills
• Experience with electronic medical records (EMR systems)
