• building a meaningful relationship with our clients
• participating in weekly music programs- like playing the drums, or dancing ballet
• community outings-like going to the library, going swimming, or visiting a pet store
• art projects- like building floats for parades, or creating Christmas cards
• health and wellness activities
• lead programming for our clients
• lead groups integrating into our wonderful communities
• assist with personal cares
• Hours: 9am-2pm, Monday-Friday

JOB SUMMARY:

The Day Service Mentor performs a number of tasks related to the fulfillment of the agency’s mission to create meaningful opportunities and community engagement for associates with diverse abilities. S/he is responsible to assist the Case Managers in carrying out the individualized program of assigned associates in such a way as to provide maximum benefit to participants.

RESPONSIBILITIES:

· Actively assist associates in the attainment of person and work goals

· Provide assistance with personal cares (eating, using restroom, etc.)

· Assist with positioning transfers

· Maintain a safe and respectful environment for associates

· Assist associates to actively engages in their community through both leisure and volunteer activities

· Respond quickly in the event of an emergency

· Preform special assignments as needed

· Communicate worker progress to all appropriate people

· Accurately record program activities on individual daily activity record

· Apply CPI behavioral intervention techniques

· Drive company vehicles

· Attend to housekeeping tasks to provide a clean and safe environment

· Fill out injury/seizure reports

REQUIREMENTS:

· Attend meetings and training regularly

· Maintain a friendly and positive attitude throughout the day

· Report all signs of illness, injury or unusual behavior to assigned Case Manager

PHYSICAL REQUIREMENTS:

· Constant physical movement throughout the day

· Standing, walking, ending, reaching, pulling, pushing at all times

· Ability to lift 20 pounds

· Ability to verbally communicate and ability to hear conversation

CERTIFICATIONS AND LECENSES:

· Certification in CPI, First Aid, CPR, and AED

· Valid drivers license with a record free from serious or frequent violations
