Duties
- Conduct psychosocial assessments for patients and families.
- Develop individualized care plans in collaboration with interdisciplinary teams.
- Provide counseling and emotional support for patients and families coping with terminal illness.
- Facilitate advance care planning (living wills, healthcare directives).
- Connect families with community resources (financial aid, housing, support groups).
- Deliver bereavement support before and after patient death.
- Ensure compliance with Medicaid, Medicare, and hospice regulations.
- Document patient progress and maintain HIPAA-compliant records.
- Collaborate with physicians, nurses, chaplains, and emergency preparedness coordinators to optimize patient experience.
Skills
- Education: Master’s degree in Social Work (MSW) preferred; Bachelor’s degree considered with relevant experience.
- Licensure: Current Michigan Social Work license required.
- Experience: 2 years in hospice, palliative care, or healthcare social work preferred.
- Skills: Empathy, crisis intervention, communication, and regulatory awareness.
- Bonus: Familiarity with payer-aligned pilots, RPM/CCM workflows, and coalition-building for compliance acceleration.
- Extensive experience in hospice care with a strong understanding of end-of-life issues and dementia or Alzheimer's disease care.
- Proficiency in child & family counseling, mental health counseling, crisis management, and working with people with disabilities or developmental disabilities.
- Knowledge of medical terminology, ICD coding systems (ICD-9/10), CPT coding, and medical records management.
- Skilled in behavioral therapy modalities such as cognitive behavioral therapy (CBT), applied behavior analysis (ABA), and trauma-informed approaches like PTSD care.
- Demonstrated ability to perform diagnostic evaluations and mental health assessments within diverse populations including pediatrics, geriatrics, and special education settings.
- Experience working with EMR systems for data collection and documentation; familiarity with HIPAA compliance standards.
- Strong case management skills with the ability to coordinate services across multiple healthcare settings including outpatient clinics and inpatient units.
- Excellent communication skills for intake interviews, patient education, family counseling sessions, and interdisciplinary team collaboration.
- Capable of crisis intervention in high-stress situations involving terminal illness or behavioral health crises; adept at working with hospice teams to provide compassionate support.
