Position Summary
Reliant Home Healthcare Services is seeking a skilled and compassionate Registered Nurse (RN) to join our team on a part-time/PRN basis. This role is responsible for performing Start of Care (SOC) assessments, supervisory visits, recertifications, discharge visits, and skilled nursing procedures including catheter insertion. The RN plays a key role in ensuring high-quality, patient-centered care in a home environment.
Key Responsibilities Start of Care (SOC) Assessments ( $140 )
- Perform comprehensive initial evaluations to determine patient eligibility, needs, and care requirements.
- Develop individualized plans of care in collaboration with the physician and interdisciplinary team.
- Initiate patient and caregiver education on home health services, treatments, and safety.
Supervisory Visits ( $100)
- Conduct required supervisory visits for home health aides and other clinical staff.
- Evaluate care delivery, ensure adherence to the plan of care, and provide coaching or corrective action when necessary.
- Document supervisory findings thoroughly and timely.
Recertification Visits ( $120)
- Complete recertification assessments every certification period to evaluate ongoing patient needs.
- Update and revise the plan of care based on patient progress and physician directives.
Discharge Visits ($120)
- Conduct discharge assessments to determine goal completion and overall patient status.
- Ensure safe transition, appropriate education, and proper follow-up recommendations.
- Complete all required documentation and discharge summaries promptly.
Catheter Insertion & Skilled Nursing Procedures
- Perform timely and skilled catheter insertions, catheter care, and troubleshooting.
- Provide wound care, medication management, disease education, IV therapy (if applicable), and other skilled nursing tasks.
- Monitor for complications and communicate changes in patient condition immediately.
Additional Duties
- Maintain accurate and compliant documentation following state, federal, and agency requirements.
- Communicate effectively with physicians, the clinical team, patients, and caregivers.
- Provide patient-centered, compassionate care while promoting safety and independence in the home environment.
Qualifications
- Active Registered Nurse (RN) license in the state of [Insert State].
- Minimum 1 year of nursing experience required; home health experience strongly preferred.
- Experience with Start of Care assessments, catheter insertion, and OASIS documentation (if applicable) highly desirable.
- Current CPR/BLS certification.
- Reliable transportation, valid driver’s license, and current auto insurance.
