*This is a private duty shift care position preferably morning shift Monday - Friday

We are seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) to join our home healthcare team. The ideal candidate will be responsible for providing high-quality patient care in home setting. As an LVN in home health, you will work closely with patients, families,

• Provide direct patient care, including administering medications and treatments
• Assist in the development and implementation of patient care plans.
• Monitor and document patient progress, reporting any changes to the home healthcare team.
• Support patients with daily living activities and personal hygiene.
• Collaborate with interdisciplinary teams to coordinate comprehensive care for patients.
• Educate patients and their families about health management
• Maintain a safe and clean environment for patients by adhering to infection control protocols.
• Participate in training programs and continuing education to enhance nursing skills.

Requirements

• Valid Licensed Vocational Nurse (LVN) license in the state of practice.
