Essential Functions:

• Administers the physical therapy treatment plan as developed by the Physical Therapist.
• Community/Resident Engagement Activities (Fall/Risk Assessments, Walking Club, Education/Exercises, Building Relationship with Community Stakeholders (Executive Directors, Activity Directors, etc.)
• Apply concepts of infection control and universal precautions in performing patient care activities.
• Accepts clinical assignments that are consistent with education and competence to care for patients.
• Uses effective interpersonal relations and communication skills.
• Meets mandatory continuing education requirements.
• Promotes compliance with all fiscal intermediaries and/or other third-party payers, though education, coaching, and other assistance as necessary.
• Demonstrates commitment, professional growth, and competency.
  
  

Qualifications

• Minimum of 1 years’ experience in Home Health.
• Homecare Homebase a plus.
• Part B experience a plus.
