Licensed Practical Nurse (LPN) – IDD Services
Location: Greenville, MS
Schedule: 7:00 AM – 3:00 PM
Position Summary
Reliable Healthcare Services is seeking a dependable and compassionate Licensed Practical Nurse (LPN) to provide one-on-one nursing care for a client with intellectual and developmental disabilities (IDD) in a home setting. This position requires hands-on clinical experience with G-tube feedings and colostomy care, along with a strong commitment to maintaining the client’s health, safety, and overall well-being.
Essential Duties and ResponsibilitiesSkilled Nursing Care
- Perform and monitor G-tube feedings, including proper setup, administration, and flushing
- Provide routine and as-needed colostomy care, including emptying, cleaning, and monitoring for complications
- Administer medications as prescribed
- Monitor vital signs and assess for changes in condition
- Report any significant changes to the RN Supervisor and/or physician immediately
Client Care
- Assist with activities of daily living (ADLs) as needed
- Promote client comfort, dignity, and independence
- Maintain a safe and clean home environment
Documentation & Compliance
- Accurately document all care provided in accordance with Mississippi Medicaid and DMH guidelines
- Follow the client’s Plan of Care (POC) and any individualized support plans
- Ensure compliance with EVV (Electronic Visit Verification) requirements
- Adhere to infection control standards and company policies
Communication
- Communicate effectively with supervisors, family members, and care team
- Participate in team meetings and provide updates on client care
- Maintain professional boundaries and confidentiality at all times
Qualifications
- Current, active LPN license in the State of Mississippi
- Current CPR/BLS certification
- Required experience with G-tube feeding and colostomy care
- Experience working with IDD population preferred
- Ability to work independently in a home setting
- Strong documentation and communication skills
Physical Requirements
- Ability to lift, turn, and reposition client as needed
- Ability to stand, walk, bend, and perform routine caregiving duties
- Ability to respond appropriately in emergency situations
Work Environment
- One-on-one care in a home setting in Greenville, MS
- Supervision provided by RN Supervisor
- Day shift hours (7:00 AM – 3:00 PM)
