About The Position
Responsible for the development and implementation of home care services for Reid Health PACE participants, including the coordination of all Durable Medical Equipment and nursing home care.
Overview Of Responsibilities
- Assesses, using the nursing process, the home care needs of a frail elderly population, and identifies and develops specific plans of care.
- Participates in the development and revision of the participant’s plan of care as a member of the interdisciplinary team.
- Coordinates the implementation of all home care services, which includes nursing, physical therapy, occupational therapy, speech therapy, and personal care services, to ensure that quality service is provided to meet client needs.
- Coordinates and authorizes all Durable Medical Equipment.
- Coordinates and supervises all nursing care for Reid Health PACE participants in the nursing home/boarding home according to program standards. Includes interaction with other facility staff and taking part in the Home Safety Assessment.
- Accesses home care services system, makes recommendations, and offers solutions to revise or modify systems to better meet participant needs.
- Actively participates in Interdisciplinary Team Meetings.
- Fosters communication between home care vendor(s) and IDT.
- Assists with pre-enrollment in-home assessments.
- Complies with CMS PACE regulations as they pertain to this role.
- Performs other duties as assigned.
Education & Experience
- Minimum Education Required: Associate of Science in Nursing (ASN)
- Minimum Education Preferred: Bachelor of Science in Nursing (BSN)
- Minimum Experience Required: 1 year of experience as RN in geriatric services
- Minimum Experience Preferred: 3 or more years of thorough knowledge of current community health nursing practice as a RN in geriatric services with 1 year of experience in supervision
- Licensure: Licensed as a RN in the State of Indiana
- Certifications: BLS, ACLS, CPR
