Overview Of Responsibilities
- Contribute to increasing participants’ satisfaction by developing and maintaining positive relationships with co-workers, participants and their families/significant others, and members of the community.
- Provide oversight, supervision, and evaluation of nursing care of participants in the Reid Health PACE Center program to promote safe, high-quality service 24/7.
- Hire, mentor and evaluate nursing staff.
- Directly or through a designee, evaluate the competence of all NCAs initially during orientation and on an ongoing basis in accordance with policy.
- Provide direct supervision of the PACE Center Nurse staff.
- Assure NCAs are being effectively supervised by the Center Manager.
- Assure timely and accurate completion of initial and periodic home care assessment by Home Care Registered Nurse and Center RN in accordance with policies and requirements of the state.
- Work collaboratively with other team members, especially the interdisciplinary team, to develop and implement the nursing and home care components of the comprehensive interdisciplinary team plan of care
- Participate in all interdisciplinary team meetings:
- Complete initial RN assessment and reassessment face to face as part of the interdisciplinary team
- Review and revises goals and approaches to participants care in coordination with interdisciplinary team.
- Assist the interdisciplinary team members in understanding the significant nursing, self-care and functional needs related to the participant’s health problems.
- Communicate with other interdisciplinary team members to coordinate follow up of nursing interventions.
- Communicate interdisciplinary team meeting information to nursing staff
- Monitor effectiveness and outcomes regularly and keep the team informed as to participant progress and status with regard to level of service.
- Ensure that assesses the participant /family understanding and capacity to manage self-care or family provided care.
- Develops and monitor the annual nursing budget to assure effective utilization of resources and recommend revisions as appropriate.
- Represent nursing as part of a quality improvement program
- Provide nursing services and supervision in accordance with State Board of Nursing requirements, nursing scope of practice, and Reid Health PACE Center policy, practice, and procedures.
- Assist in developing and maintaining written job descriptions for nursing personnel, and assigning duties based on education, training, competencies, and job descriptions.
- Participate in family meetings, staff meetings, in-service and training programs, staff orientation and education activities, rehab services meetings and Quality Improvement activities.
- Follow all PACE Program Policies and Procedures and Occupational Safety and Health Administration (OSHA) safety guidelines.
- Protect privacy and maintain confidentiality of all company procedures and information about employees, participants and families.
- Practice standard precautions and follows Reid Health PACE Program Infection Control protocols.
- Participate in continuing education classes and any required staff and training meetings.
- Maintain professional affiliations and any required certifications.
- Perform other duties as required or requested.
- Establish and oversees an effective 24/7 “on-call” process in collaboration with the Medical Director for triage, nursing and medical coverage and participate in providing coverage as appropriate.
- Establish, supervise and evaluate the operations of the Center and work collaboratively with the Medical Director to assure services are of the highest quality and delivered efficiently and effectively
Education & Experience
- Minimum Education Required: ASN
- Minimum Education Preferred: MSN
- Minimum Experience Required: 3 years of experience in nursing leadership with experience in geriatrics, health care and home health.
- Minimum Experience Preferred: 3 years of experience working with the geriatric population.
Licensure & Certification
- Licensure: License and current registration to practice as a Registered Nurse in Indiana
- Certifications: CPR, PPD
