Overview Of Responsibilities
- Coordinates and implements the PACE program.
- Continuously evaluates the participants' changing needs and makes necessary program adjustments. Recommends changes in service delivery and staffing as required by the development and growth of the agency.
- Directs, supervises, and evaluates the performance of direct reports.
- Ensures that the job responsibilities, authorities and accountability of all direct reports are defined and understood.
- Responsible for maintaining compliance with all local, state and federal program requirements; obtains and maintains certification under Centers for Medicare and Medicaid Services (CMS) as well as any other state or federal law or rules.
- Assists in program development and preparation of Reid Health PACE Center’s short-range and long-term plans, budgets and related information systems based on broad organization goals and growth objectives.
- Leads Reid Health PACE Center Interdisciplinary Team (IDT) and acts as facilitator of IDT daily meetings and plan of care meetings.
- Coordinates the Interdisciplinary Team’s care management process to ensure best decisions are made regarding the participant’s changing condition.
- Ensures the coordination with home health, rehabilitative, long-term care and medical/specialty services according to participant care plans.
- Assists Director in development of annual operational and capital budgets. Ensures financial benchmarks are met.
- Assists in development and implementation of Reid Health PACE Center operational policies, procedures, protocols and regulations for direct participant care and support services.
- Monitors operations to ensure consistent compliance with established quality assurance standards.
- Assists Director with evaluation of Reid Health PACE Center performance against national results and established service standards, and budget and other financial criteria.
- Assists in development and implementation of performance improvement plans as indicated by evaluation of it.
- Participates regularly in all quality assurance/quality improvement committees as part of the overall quality improvement program.
- Establishes positive working relationships with community health and social service providers.
- Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications. Provides appropriate in-service education.
- Complies with CMS PACE regulations as they pertain to this role.
- Provides administrative on-call coverage for the Reid Health PACE Center on a rotation basis.
- Performs other duties as required or requested.
Education & Experience
- Minimum Education Required: Bachelor’s degree in health or human services field.
- Minimum Education Preferred: Master’s degree preferred.
- Minimum Experience Required: A minimum of five (5) years of health care experience required; a minimum of one year experience in working with the frail or the elderly population. The PACE Center Manager shall be knowledgeable of the physical, mental, and social needs of the elderly.
- Minimum Experience Preferred: 3-5 years of experience in a geriatric care setting with progression of responsibility. Prior involvement in the start-up of healthcare related business.
