About the Role:
Position Summary
The Private Duty Nurse provides individualized, one-on-one skilled nursing care to pediatric and adult patients in their home or community setting. This role requires a compassionate, highly skilled clinician capable of working independently while adhering to the New York State Nurse Practice Act, agency policies, and the patient's individualized Plan of Care (POC). The nurse serves as a primary clinical resource for patients, families, and caregivers across both pediatric and adult populations.
Key Responsibilities
Clinical Care
- Perform comprehensive nursing assessments and ongoing monitoring for pediatric and adult patients
- Administer medications, IV therapy, tube feedings (G-tube/J-tube/NG-tube), wound care, and other skilled nursing interventions per physician orders
- Operate and monitor complex medical equipment, including ventilators, suction machines, feeding pumps, pulse oximeters, and cardiac monitors
- Implement and update the individualized Plan of Care in collaboration with the interdisciplinary team
- Respond promptly to changes in patient condition; notify physician and clinical supervisor as appropriate
- Provide tracheostomy care and airway management as ordered
- Perform catheter care, ostomy care, and other complex nursing procedures as required
Required
- Active, unrestricted Registered Nurse (RN) or Licensed Practical Nurse (LPN) license issued by the New York State Education Department (NYSED)
- Current BLS/CPR certification (American Heart Association or American Red Cross)
- Minimum 1-2 years of clinical nursing experience; pediatric or home care experience preferred
- Ability to work independently with minimal direct supervision
- Strong written and verbal communication skills
Preferred Qualifications:
- Experience working in a clinical or hospital setting.
- Additional certifications such as IV therapy, wound care, or geriatric nursing.
- Familiarity with electronic health record (EHR) systems.
- Ability to speak a second language relevant to the patient population served.
- Continuing education or specialized training in areas such as chronic disease management or patient education.
Responsibilities:
- Provide basic nursing care to patients, including monitoring vital signs and reporting changes in patient condition.
- Administer prescribed medications and treatments accurately and safely according to established guidelines.
- Assist patients with daily living activities such as bathing, dressing, and mobility support.
- Collaborate with registered nurses and physicians to implement patient care plans and document patient progress.
- Maintain a clean and safe environment by following infection control procedures and healthcare regulations.
- Educate patients and their families on health maintenance, disease prevention, and post-treatment care.
- Prepare and maintain accurate patient records, including observations, treatments, and medication administration.
