We’re Hiring: RN Nurse Case Manager

At Rehabilitation Management & Consulting, our team of highly experienced Rehabilitation Nurses provides ongoing support, education, and medical care coordination to individuals navigating significant or catastrophic injuries. We focus on true advocacy—empowering patients through the rehabilitation process to maximize their recovery.
We are looking for an empathetic, organized Case Management Nurse to serve our clients across West, Central, and Northern Michigan!
Pay & Schedule
- Compensation: $40.00 – $45.00 / hour (commensurate with experience)
- Travel Compensation: Paid drive time AND mileage reimbursement
- Hours: 30–40 hours/week
- Schedule: Monday – Friday, 9:00 AM – 5:00 PM (No nights, weekends, or holidays!)
- Location: Remote / Hybrid (Must live within a 50-mile radius of Greater Grand Rapids, MI for local client visits)
What You’ll Do
- Patient Advocacy & Care: Coordinate medical care and long-term recovery plans for individuals with complex, catastrophic injuries.
- Regional Support: Travel locally to visit clients across West, Central, and Northern Michigan, ensuring personalized, face-to-face care.
- Continuum of Care: Oversee discharge planning and utilization management to ensure seamless transitions between care settings.
- Empowerment & Education: Provide hands-on guidance and education to patients and families throughout their rehabilitation journey.
- Multidisciplinary Coordination: Collaborate directly with physicians, therapists, families, and insurance providers to optimize patient outcomes.
What We’re Looking For
- Active RN License (Associate’s or Bachelor’s degree in Nursing).
- Preferred Experience: Rehabilitation nursing, home care, discharge planning, or nurse case management
- Strong background in clinical assessment, utilization management, and complex care coordination.
- Excellent communication, organizational skills, and the ability to work independently.
- Residency: Must reside within 50 miles of the Greater Grand Rapids, MI area.
