Nurse Liaison / Licensed Practical Nurse
Job Duties
Overview:
Process all referrals to ensure the patient meets criteria and will benefit from rehabilitation services and to facilitate effective transitions into the rehabilitation setting. The liaison in collaboration with the physician, will determine readiness for program participation and those services recommended are appropriate, reasonable and necessary.
Qualifications:
Graduate of an accredited school of nursing with current Louisiana Licensure as a Licensed Practical Nurse.
Responsibilities:
Nurse Liaison Duties:
- Interview and prescreen all referrals.
- Review evaluations with physician and director of nursing for acceptance of referrals.
- Obtain copies of old records for charts.
- Copy and label medical charts for data entry.
- Enter data on all referrals into E-Rehab data program.
- Research and request pertinent medical information for rehabilitation diagnosis for admission.
- Verify private and supplemental insurance benefits.
- Verify Medicare benefits and days.
- Verify Medicaid benefits.
- Verify insurance information with patient/family.
- Coordinate with the physician, director of nursing and HIM for codes and rehabilitation diagnosis.
- Initiate pre-authorizations for private insurance, Medicaid and Medicare replacement insurance.
- Submit clinicals and all required documents to authorization department to facilitate the authorization processes.
- Follow up with insurance authorization department/case manager to ensure and verify clinicals were received and process is initiated.
- Obtain and record authorization information for admit.
- Coordinate with case management for diagnostic testing prior to admit as needed and report results to physician and director of nursing.
- Coordinate and follow up with case management, physician and family regarding patient admissions and transfers.
- Update and maintain patient status on E-Rehab data program through the admit process.
- Maintain patient information board for communication to all departments.
- Educate potential patients and/or family on policies and procedures of inpatient rehabilitation
- Address all questions and concerns for the patient/family prior to admit.
- Obtain consents from the patient /family prior to admission.
- Verify medical information with patient, family and medical staff.
- Coordinate with case management to follow up with private insurance, Medicaid and Medicare replacement insurance.
- Maintain ongoing rapport with case management, nursing staff at outside facilities and doctor’s offices.
- Coordinate with rehabilitation staff and therapy for transfer of admissions.
- Coordinate with purchasing department for special equipment or supplies for potential admits.
- Coordinate with unit clerk, nursing, director of nursing and physician on time of admission.
- Obtain discharge orders from transferring facility for nursing staff.
- Maintain and update Medicare and Medicaid websites for access.
- Maintain mileage/expense sheet for home evaluations or hospital evaluations.
Nurse Liaison will be in on call rotation. Call is a week at a time from Tuesday to the following Monday. When on call the nurse will cover the floor for any LPN call ins.
