Role Description This part-time Physical Therapist role is a hybrid position based in Langhorne, PA. You will evaluate patients with complex neurological and rehabilitation needs, develop individualized care plans, and deliver evidence-based physical therapy interventions in home and community settings. Daily activities include performing functional assessments, planning and progressing therapeutic exercises and mobility training, and educating patients and caregivers on safety, equipment use, and home exercise programs. You will collaborate closely with an interdisciplinary team to align goals, participate in team meetings, and ensure continuity of care. The role also includes maintaining accurate, timely clinical documentation and communicating regularly with the interdisciplinary team as needed.
Qualifications
- Strong clinical foundation in Physical Therapy and Rehabilitation, with experience or interest in neurologic and high-acuity populations.
- Demonstrated skills including performing assessments, developing treatment plans, and monitoring patient progress.
- Ability to provide safe, effective Home Care, including in-home evaluations, equipment and safety recommendations, and caregiver training.
- Current Physical Therapist license in the state of Pennsylvania and graduation from an accredited Physical Therapy program.
- Effective communication, documentation, and time-management skills, with the ability to work both independently and as part of an interdisciplinary team.
- Valid driver’s license, reliable transportation for community and home visits, and comfort working in varied home and community environments.
- Previous experience in neuro rehabilitation, home health, or community-based rehab is preferred but not required.
Salary Range $55-$62/hr plus paid travel and mileage
