About the RoleRSI is looking for a compassionate and motivated Transition Specialist to support students with disabilities ages 14–21 as they prepare for life after high school.
This hands-on position focuses on career exploration, workplace readiness, work-based learning, job coaching, self-advocacy, and employment support. The specialist works closely with students, families, schools, employers, Vocational Rehabilitation, and community partners to help students build skills, gain experience, and reach their goals.
Responsibilities- Provide individualized transition and employment support.
- Help students explore careers and develop workplace skills.
- Coordinate job shadows, work experiences, and other community-based opportunities.
- Provide job coaching and support in workplace settings.
- Collaborate with families, schools, employers, and service providers.
- Maintain documentation and track student progress.
- Assist with group lessons, student activities, and community outreach.
A degree or experience in social work, human services, education, rehabilitation, counseling, employment services, or a related field is preferred, but not required.

Strong communication, organization, reliability, and the ability to build positive relationships are important. A valid driver's license and reliable transportation are required.
