Primary Care Select is a family-owned healthcare company seeking a compassionate and motivated Nurse Practitioner to join our team serving Skilled Nursing and Long-Term Care facilities in Alabama.
Our providers play a vital role in improving patient outcomes, supporting rehabilitation progress, and reducing unnecessary hospitalizations through responsive onsite care.
- Provide post-acute and long-term care services in Skilled Nursing Facilities
- Conduct assessments, medication reviews, and routine follow-up visits
- Collaborate closely with physicians, nursing leadership, and facility staff
- Serve as a primary clinical resource for residents and families
- Help guide rehabilitation and discharge planning
- Active Alabama NP and RN licensure
- National Board Certification
- Experience or background in adult, family, internal, or geriatric medicine preferred
- Ability to work independently in a collaborative environment
- Willingness to occasionally travel to sister facilities in Alabaster and Talladega
