What are the responsibilities and job description for the Perinatal Social Worker position at Regional One Health?
A Brief Overview
The Perinatal Social Worker, Maternal One Model provides and develops holistic, individualized plans and interventions/strategies that address clients' psychosocial needs, including screening, education, counseling, and referrals to alleviate social risk factors, barriers to resources and improve clinical outcomes.
What you will do
- Utilizes admission data, medical records, provider feedback and interviews to assess client’s psychosocial needs to determine program appropriateness.
- Consents and onboards qualifying clients into program.
- Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.
- Participates in case reviews to evaluate individualized client management and progress.
- Consults with healthcare and program team members to promote, monitor, and evaluate compliance with client’s plan.
- Actively participates in program and multidisciplinary client rounds to report updates/ outcomes on individualized education care plans.
- Assists with discharge planning and ongoing internal and external processes for client group.
- Identifies appropriate resources, including but not limited to transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of client program plan.
- Supervises and/or precepts social work students as appropriate.
- May perform insurance screenings and assist clients with Safety Net applications when needed.
- Collaborates with program manager to ensure psychosocial interventions produce specified results in the monthly/ annual work plan(s) to the required standard of quality and within the timeline and budget parameters.
- Ensures that the program progresses in accordance to its obligations and complies with regulations and organizational policies.
- Participates in outreach, client recruitment, and community events as determined by needs of Program.
- Engages in professional development activities to maintain and enhance expertise.
- Master's Degree Social Work from an accredited college or university. Required
- LMSW - Licensed Medical Social Worker - State Licensure Licensed Master of Social Work (LMSW) in the State of Tennessee Upon Hire Required or
- LCSW- License Clinical Social Worker Licensed Clinical Social Worker (LCSW) in the State of Tennessee Upon Hire Required
- Minimum 3 years experience Clinical experience in hospital setting. Required
- Perinatal background. Preferred
- Fluent, multilingual, especially in Spanish, is beneficial.
- Prior experience in grant-funded projects beneficial.
