What are the responsibilities and job description for the Nurse Liaison position at Regional One Health?
Regional One Health Extended Care Hospital is currently seeking a Nurse Liaison.
A Brief Overview
Conduct patient assessments to identify patients for potential admission into the system. Responsible for managing the referrals and admissions that are generated through their assigned hospitals and territories by representing the hospital with professionalism to referral sources. Designs and manages the market strategies for their assigned hospitals. Conduct one on one contacts with potential referral sources, maintain positive relationships with current referral sources. Utilizes educational opportunities to present the hospital to referral sources. Identifies, contacts and manages potential clients. Sources potential, appropriate patients for hospital. Coordinates transmission of clinical and benefit treatment to patients, families and outside agencies. Acts as liaison for outside agencies, non-plan facilities, and outside providers.
What you will do
- Participate in quarterly Clinical Quality Collaborative (CQC) reviews with referring hospitals executive administration and quality officers to brand ROH ECH, develop partnerships, and identify patient clinical outcomes.
- Attend rounds at responsible hospitals & territories as permissible by administration to establish referral patterns for patients identified as meeting LTACH criteria.
- Facilitates the timely and appropriate completion of medical records, transfers, and forms for admission.
- Maintains follow-up and coordinates with patient/family/hospital departments regarding all aspects of patient care needs for admission (i.e., patient transfer, medical records, specialized equipment, etc.).
- Interacts and communicates in a customer responsive and professional manner. Works collaboratively and coordinates activities with hospital/medical staff and community agencies to integrate and implement activities.
- Participates in monitoring/reviewing quality improvement and utilization review activities. Serves as contact person regarding all inquiries and reports.
- Prepares and maintains required statistics, reports, records and files associated with business development..
- Coordinates market research projects as requested.
- Participates on patient progress, interdisciplinary team, daily admission review & other hospital committees as requested.
Qualifications
- Graduate of an approved nursing school/program Required and
- Bachelor's Degree BSN degree Preferred and
- Qualified by education, training or experience to work with the neonate, infant, toddler, pre-schooler, school-age, adolescent, adult and/or geriatric patient population as specialty assignment dictates Required
- RN - Registered Nurse - State Licensure and/or Compact State Licensure Current licensure in TN as RN Upon Hire Required
- Minimum 2-3 years of clinical experience in an acute care hospital and/or case management experience. Required and
- Two years of marketing/sales experience. Preferred
- Experience and skills applicable in area of assignment Required
