Job Overview
Regional Hospice is a well-established hospice with 3 offices covering the southwest corner of Missouri---Lebanon, Springfield, and Joplin.
We are seeking a compassionate Hospice Nurse Practitioner to join our dedicated team. In this role, you will perform regulated Face to Face visits to assist the Medical Director and Interdisciplinary Team to recertify the patient for hospice services.
This job is paid per visit with mileage reimbursement.
Duties
- Perform scheduled Face to Face visits with patients who live in private homes, assisted living facilities, long term care facilities, etc.
- You will receive a list of required visits for the month and have 30 days to complete them and document your visits.
- You make your own schedule as long as each visit is completed timely based on that patient's cert period dates.
- The clinical findings of that visit will be provided to the certifying physician, for use in determining whether the patient continues to have a life expectancy of 6 months or less, should the illness run its normal course
- Occasionally you would be called upon to do a Face to Face visit in the same/next day for an urgent admission.
- Maintain accurate medical documentation in compliance with HIPAA regulations via a web-based EMR.
Qualifications
- Current Nurse Practitioner license with a knowledge of hospice philosophy, KPS, PPS, FAST, etc preferred.
- Willing to train the right candidate.
- Familiarity with EMR/EHR systems.
- Experience working in various settings including nursing homes, assisted living facilities, home care environments, or outpatient clinics.
