Registered Nurse Case Manager- Part Time
Join our non-profit, community based Hospice and Palliative Care team to truly make a difference in the life of others!
Are you looking for a return to a meaningful patient centered career?
Our RN staff conduct one on one weekly visits with patients and families in their home setting to provide compassionate end of life care. The RN is a part of an interdisciplinary team to provide the best care and outcomes for the patients served in our local communities.
Candidates must have strong clinical and patient/family relationship skills and be willing to travel to provide case management and care to patients in our Ashland service area.
