Information & Access Specialist
Full-Time (37.5 hours/week) | Saint Joseph, MI | Salary: $40,000 - $46,000
Flexible schedule, 14.5 paid holidays, excellent benefits, and meaningful work serving Southwest Michigan. Some hybrid work days may be available after successful orientation and demonstrated independent performance.
About the Role
Region IV Area Agency on Aging (RIV AAA) is seeking an Information & Access Specialist to serve as a primary point of access for older adults, caregivers, adults with disabilities, and individuals with health-related social needs.
This role helps people navigate community resources, healthcare services, benefits, and support programs while providing person-centered guidance and problem-solving. The position combines office-based work with regular community office hours throughout Berrien, Cass, and Van Buren Counties.
Responsibilities
- Provide information, options counseling, and resource navigation for individuals, caregivers, and community partners.
- Conduct health-related social needs screenings and help individuals connect with services and supports.
- Coordinate referrals and warm handoffs to RIV AAA programs and community resources.
- Provide follow-up support to help individuals successfully access recommended services and address barriers.
- Represent RIV AAA at community locations, outreach events, and partner sites throughout the region. This position does require some travel.
Qualifications
- Bachelor's degree in Human Services or related field, or an equivalent combination of education and experience.
- Experience working with older adults, individuals with disabilities, caregivers, healthcare organizations, community-based organizations, or related fields.
- Ability to assess needs, solve problems, and assist individuals in navigating services and resources.
- Strong communication, interviewing, and relationship-building skills.
- Strong customer service skills, computer proficiency and ability to accurately document services and participant interactions.
