We’re inviting an experienced and mission-driven Advanced Practice Provider (NP or PA) to join our Integrated Care at Home team. What You’ll Do
· Conduct in-home clinical assessments and medication reconciliation
· Develop and regularly update Plans of Care in partnership with patients and caregivers
· Diagnose, treat, and manage acute and chronic conditions within scope of practice
· Educate patients and caregivers on conditions, medications, and symptom management
· Collaborate with the Medical Director, interdisciplinary team, and community partners
· Maintain strong clinical documentation and adherence to compliance and continuous quality

Qualifications
Required:
· Current licensure as a Nurse Practitioner or Physician Assistant in the State of Michigan
· Practice independently with strong assessment, diagnostic, and care-planning skills
· Excellent communication & relationship-building abilities with patients, families, and teams
· Valid driver’s license with ability to travel throughout Berrien, Cass, and Van Buren counties
Preferred:
· Experience providing care in home-based, community, or adult foster care settings
· Background in primary care, geriatrics, chronic disease management, or integrated care
