Community Support Worker

Key Responsibilities
- Provide emotional and social support.
- Connect clients to community resources and services.
- Develop personalized care plans to support individuals in reaching their goals.
- Advocate for clients in areas like healthcare and housing.
- Offer crisis intervention and conflict resolutions and maintain accurate records.
- Work with clients across diverse settings while coordinating with professionals.
What We're Looking For
- Education/Experience: Bachelor’s degree in human services preferred; relevant experience considered.
- Skills: Strong interpersonal abilities, empathy, and a dedication to improving lives.
- Valid driver's license and willingness to travel
