You'll be a great fit if:
- You have experience Connecting with the community
- You have experience Coordinating Community Events and Health Education Materials
- You have experience advocating for community needs and resources by attending community meetings and events.
- You have experience managing a caseload of patients.
- You have experience providing health education.
- You can read, write, and maintain patient records and document using computer systems.
- You can help patients feel comfortable in addressing their social drivers of health.
- You can interact positively with a diverse patient population, of all ages, backgrounds, and personalities.

Requirements
To be Considered You'll Need:
- High School Diploma or equivalent education required.
- Post-secondary degree in related studies (i.e. Public Health, social work, psychology, or related human services field) (Preferred)
- Community Health Worker Certified (Preferred)
Salary : $40,000 - $45,000
