Your Impact
- Empower patients and families to take control of their health
- Break down barriers to care, including social determinants of health
- Serve as a bridge between the community and healthcare providers
- Build trust within diverse populations through culturally competent care
- Improve health outcomes through education, advocacy, and outreach
- Engage directly with community members through outreach and events
- Provide health education and informal counseling to patients and families
- Help patients navigate healthcare systems and access community resources
- Support care teams with patient follow-ups and coordination
- Maintain accurate documentation in electronic systems
- Advocate for patients’ needs at both individual and community levels

- Bilingual in English and Spanish (required)
- High School Diploma or equivalent (required)
- Degree in Public Health, Social Work, Psychology, or related field (preferred)
- Community Health Worker Certification (preferred)
