Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patient health by taking vital signs and observing for any changes in condition
- Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary
- Administer medications following care plans and HIPAA regulations to ensure safety and compliance
- Support clients during outings or community activities to promote social engagement and independence
- Maintain accurate documentation of care provided, patient observations, and progress notes
- Use specialized equipment such as Hoyer lifts or wheelchairs to ensure safe transfers and mobility assistance
Requirements
- Prior experience in group homes, assisted living facilities, nursing homes, or home & community care settings is highly preferred
- Knowledge of patient care techniques including CPR, first aid, Alzheimer's and dementia care, autism support, and hospice care
- Ability to work with individuals with disabilities or developmental challenges with patience and empathy
- Strong observational skills for patient monitoring and recognizing health or behavioral changes
- Physical ability to perform heavy lifting and assist with mobility tasks safely
- Familiarity with HIPAA regulations to protect client confidentiality and privacy
- Valid certifications in CPR and first aid; additional certifications in medication administration or nursing assistance are a plus
- Excellent communication skills to collaborate effectively with team members, clients, and families
- Class E Chaffeur's drivers license

- DSP: 3 years (Preferred)
License/Certification:
