Job Type: Contracted, Part-Time
Schedule: Self-determined – you can choose to set your schedule for:
Weekdays only (Monday-Friday)
Weekends only (Saturday & Sunday) or no weekends
Day or evening shifts
Location: On the road; locations will vary (in-person at the home(s) of client(s) living in and around the Austin area)
Qualifications and skills include:
REQUIRED: Possess a current PT license to practice in the state of Minnesota
REQUIRED: Valid Driver's License and reliable transportation
Ability to work independently
Responsibilities Include:
Evaluate and assess the patient’s current physical abilities and limitations and develop individualized treatment plans based on the patient’s needs
Provide physical therapy treatments and therapeutic exercises to clients in their homes
Monitor and document patient’s progress
Educate patient and family on how to manage and care for the patient’s physical limitations
Communicate with other healthcare staff to coordinate care for the patient
