Job Overview
We are seeking a dedicated and compassionate *PER DIEM* Field LVN for NORTHRIDGE / CHATSWORTH, CA to join our team. In this role, you will provide high-quality nursing care to patients in various settings, including their homes and community environments. Your expertise will help ensure that patients receive the necessary medical attention and support for their health needs. The ideal candidate will possess a strong clinical background and be skilled in patient care management.
Responsibilities
- Provide direct nursing care, including wound care, medication administration
- Assist with patient education on disease management, medications, and lifestyle modifications.
- Monitor and document patients’ progress in accordance with care plans.
- Collaborate with interdisciplinary healthcare teams to ensure comprehensive patient care.
- Maintain accurate and timely documentation in DSL
- Ensure compliance with infection control protocols and healthcare regulations.
- Provide support for family and caregivers regarding patient care and treatment plans.
- Assist with mobility and activities of daily living (ADLs) as needed.
Skills & Qualifications
- Valid California LVN license and BLS certification required.
- 1 year of experience in home health, hospice - highly desirable.
- Strong clinical skills in patient assessment, wound care, and medication administration.
- Familiarity with infection control practices and patient safety protocols.
- Ability to work independently while effectively communicating with the care team.
- Comfortable using electronic health records DSL
- Compassionate and patient-focused with excellent interpersonal skills.
