Duties
- Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform personalized care plans.
- Document all patient interactions accurately using electronic health record (EHR)
- Coordinate with multidisciplinary teams to develop and implement effective care strategies tailored to individual patient conditions.
- Wound care, diabetic management, medication management.
Pay: $40.00 - $45.00 per hour
Expected hours: 3.0 per week
