Regalcare of Holyoke is a skilled nursing and rehabilitation center committed to delivering high-quality, resident-centered care and strong clinical outcomes. We are currently seeking an experienced, detail-oriented MDS Coordinator to join our leadership team in a full-time role.
What You’ll Do:
- Coordinate and complete accurate, timely MDS assessments in compliance with state and federal regulations
- Manage the RAI process and support interdisciplinary care planning
- Ensure documentation supports clinical outcomes and reimbursement
- Collaborate with nursing, therapy, and administrative teams
- Monitor assessment schedules, audits, and quality measures
- Support surveys and quality improvement initiatives
- Active RN or LPN license in good standing
- Prior MDS experience in a long-term care setting preferred
- Strong knowledge of Medicare/Medicaid guidelines and RAI process
- Excellent organizational and time-management skills
- Ability to work independently and meet deadlines
