Responsibilities:
- Coordinate the MDS assessment process, including scheduling and facilitating assessments with interdisciplinary teams
- Ensure accurate and timely completion of MDS assessments, RAPs, and other required documentation
- Maintain accurate and up-to-date patient and resident records, including medical and social histories
- Collaborate with medical teams, families, and other healthcare professionals to ensure comprehensive care
- Develop and implement effective communication strategies to ensure seamless coordination of patient and resident care
- Identify and address any discrepancies or inconsistencies in patient and resident records
- Participate in quality improvement initiatives to improve patient and resident outcomes
- High School Diploma or equivalent required
- MDS Coordinator experience preferred
- Strong organizational and analytical skills, with attention to detail and ability to work in a fast-paced environment
- Excellent communication and interpersonal skills, with ability to work effectively with diverse stakeholders
- Ability to maintain confidentiality and handle sensitive information
