- Respond promptly to weekend referrals from hospitals and other sources
- Review clinical documentation to determine admission eligibility
- Coordinate with clinical and administrative teams to facilitate smooth admissions
- Communicate effectively with families regarding admission process and expectations
- Maintain accurate documentation and tracking of referrals
- Represent the facility professionally with referral partners
- Experience in healthcare admissions, case management, or marketing preferred
- Knowledge of skilled nursing or long-term care a plus
- Strong communication and multitasking skills
- Ability to work independently and make timely decisions
- Reliable transportation for local travel within Boston (if applicable)
- Weekend schedule (specific hours to be determined)
