Responsibilities
- Perform comprehensive patient assessments to determine infusion needs, vital signs, and overall health status.
- Administer IV infusions, medications, and therapies following strict aseptic techniques and established protocols.
- Educate patients and their families on medication administration, infusion procedures, and safety measures to promote adherence and comfort.
- Monitor patients for adverse reactions or complications during infusion treatments, responding swiftly with appropriate interventions.
- Maintain accurate medical documentation, ensuring compliance with HIPAA regulations.
- Coordinate discharge planning and follow-up care to support ongoing health management in the home setting.
- Collaborate with specialty pharmacies to optimize patients experience outcomes across diverse clinical scenarios.
Experience
- Proven nursing experience in home infusion therapy or acute care settings such as ICU, emergency medicine, or hospital medicine is highly preferred.
- Prior home or community-based nursing experience demonstrates your adaptability to outpatient settings and patient-centered care.
- Knowledge of home based documentation and communication.
- Experience working with individuals with disabilities, developmental disabilities, or memory care needs will support diverse patient populations.
- Strong understanding of medication administration protocols, and aseptic techniques ensures safe practice standards.
- Additional skills in IV insertion/venipuncture, central line care and maintenance, and patient education and teaching are advantageous to provide comprehensive home infusion services.
