Job Summary
As a Hospice Agency Coordinator, you will play a pivotal role in ensuring that our patients receive compassionate and comprehensive end-of-life care. What you’ll do
- Coordinate patient admissions and discharges to ensure seamless transitions in care
- Collaborate with healthcare professionals to develop individualized care plans tailored to each patient’s needs
- Schedule and manage appointments for interdisciplinary team members, including nurses, social workers, and chaplains
- Serve as a primary point of contact for patients and families, providing updates and addressing concerns with empathy and clarity
- Maintain accurate patient records and documentation in compliance with regulatory standards
- Facilitate communication between patients, families, and the hospice team to promote understanding of services offered
- Assist in training new staff members on agency protocols and procedures
- Participate in quality improvement initiatives to enhance patient care services
Basic qualifications
