Role Description
This is a full-time on-site role for a Hospice Liaison at our Pleasanton CA, location. The Hospice Liaison will build and maintain relationships with healthcare providers, hospitals, and referral sources to promote Redwood Hospice services. Key responsibilities include conducting outreach efforts, educating the community about hospice and palliative care, and engaging potential patients and caregivers. Additional duties include coordinating admissions, assessing patient needs, and ensuring smooth transitions to hospice care while upholding the highest standards of compassion and professionalism.

Qualifications
- Experience or knowledge in Hospice Care, End-of-Life Care, and Palliative Care
- Skillset in Sales with the ability to build strong relationships and generate referrals
- Background in Home Care coordination is beneficial
- Excellent interpersonal and communication abilities to engage with diverse stakeholders
- Strong organizational and time management skills
- Relevant clinical or healthcare experience is a plus
- Bachelor’s degree in healthcare administration, nursing, or a related field preferred
- Ability to travel locally and maintain a valid driver’s license
